Safeguarding and Terms & Conditions
Before any initial treatment, you will be asked to sign and date this consent form which will then be reviewed every six months. Please read terms and conditions below.


All treatments are governed by conditions of practice put in place on my registration at the request of my regulator the Health and Care Professions Council.
1. You must confine your professional practice as a Physiotherapist to adult male service users over the age of 18 years. You must not in your capacity as a Physiotherapist provide any clinical services to any female service user.
2. You must not carry out professional work in a private residence setting including your home and also including visits to a service user's private residence.
3. You must place yourself and remain under the indirect supervision of a supervisor, preferably registered by the HCPC, or another appropriate statutory regulator if this is not possible, and supply details of your supervisor to the HCPC within 28 days. You must attend upon that supervisor as required and follow their advice and recommendations.
4. You must meet with your supervisor on a monthly basis to demonstrate compliance with the interim conditions of practice. At least once every three months you must have a physical in-person meeting with your supervisor.
5. You must allow your supervisor to provide information to the HCPC about your compliance with the interim conditions of practice 3 and 4 and a report signed off by the supervisor shall be submitted to the HCPC by you, at least one week prior to any review hearing to demonstrate compliance.
6. You must inform the HCPC within seven days if your rental agreement with the psychotherapy practice ends.
7. You must inform the HCPC within seven days if you take up any other or further professional work as a Physiotherapist including work outside of the UK.
8. You must inform the HCPC within seven days of returning to practice in the United Kingdom.
9. You must inform the HCPC within seven days of becoming aware of:
A. any further patient complaint you are involved in;
B. any investigation started against you; C. and any disciplinary proceedings taken against you.
10. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work (which would include all patients receiving Physiotherapy services from you in a self-employed capacity); male service users receiving any clinical services from you must, before receiving such services, be provided with a written copy of these conditions. A written text of the entirety of these Conditions must appear on your website.
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application).
11. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with:
A. any organisation or person employing or contracting with you to undertake professional work (which would include all patients receiving Physiotherapy services from you in a self-employed capacity);
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application).
12. You will be responsible for meeting any and all costs associated with complying with these conditions
